Gowan Consulting offers occupational therapy services to people and organizations that are looking to create or sustain healthy workers and workplaces. Gowan Consulting provides services to people at their work location, or virtually when appropriate. In providing services to individuals, Gowan Consulting complies with the Personal Health Information Protection Act, 2004, which governs the collection, use and disclosure of personal health information in Ontario.
Information Collection
Those engaging with Gowan Consulting as customers or clients agree to have representative(s) of Gowan Consulting collect, use, and disclose their personal information for the purposes of providing occupational therapy services. Informed and knowledgeable consent from customers and clients is required for Gowan Consulting to commence services.
Information obtained is gathered and used to:
- Provide occupational therapy
- Obtain payment
- Manage quality, risk, and care in the services provided
- Compile aggregate data on service delivery
- Comply with laws and regulatory obligations
- Conduct analysis for the betterment of the customer and client experience
Information Release
With permission, the personal health information of occupational therapy clients may be shared within their circle of care, including to funders and other health providers.
Record Access and Accuracy
Customers and clients are to request access to their occupational therapy records directly from the funder of the services. Should the funder not provide them, Gowan Consulting will release them as per governing requirements. Gowan Consulting will abide to all circumstances whereby the release of client or customer personal or private information is required via regulators, or through court order or legal proceeding.
Customers and clients have a right to have accurate records and are encouraged to communicate concerns or discrepancies to Gowan Consulting.
Privacy Breach
Gowan Consulting commits to notifying all parties should private and personal information be compromised. Breaches will be contained and resolved expediently and with care.
Website Use and Security
We take your use of our website seriously. While we endeavor to protect all personal information obtained by us as we conduct our business, the company cannot ensure or warrant the security of any information you transmit via the internet. In using our site, you accept this risk. Further, we can only govern our website, and not others you may access through links our site contains.
Under 16: If you are under 16, we have a special obligation to protect you from submitting identifiable information. Any information submitted online by a minor will be deleted.
The personal information of website visitors is not obtained. However, information such as browser type, operating system, and user IP address is gathered to monitor user online experience.
Users may submit personal information through the website for the purposes of submitting a referral or engaging in promotional activities. In these cases, all information gathered will be used by the Company only as necessary for legitimate business interests, including to provide products or services, or to improve the website.
